The editable PDF of the In-Training Assessment (ITA) form can be found on our Learning Portfolio webpage. Trainees and Supervisors of Training (SoTs) can download the form and edit the ITA form without needing to print out a copy. The old non-editable version of the ITA form is still available on the Learning Portfolio webpage for any trainees who would prefer to fill out the form manually.
